Vitamin D originating from dietary and endogenous sources is converted to 25-hydroxyvitamin D in the liver and subsequently to 1,25-dihydroxyvitamin D in the kidney. Measurement of both the 1,25-dihydroxy and 25-hydroxy forms may be useful for determining determining vitamin D status in patients with renal failure and for differential diagnosis of 1) primary, secondary, and tertiary hyperparathyroidism, 2) hypercalcemia, 3) type I rickets, and 4) type II rickets.

Fasting preferred, but not required
Collect blood in a standard red-top serum Vacutainer(R) tube.
Allow blood to clot at room temperature. Centrifuge and separate the
serum from the cells immediately. Alternatively, collect blood in a
serum separator tube, allow to clot at room temperature. Centrifuge
and remove from the gel within 48 hours.
