For evaluation of female infertility. LH surge and mid-luteal progesterone are used as indicators of completion of ovulation. FSH, prolactin, progesterone and DHEA-S are used to differentiate hypergonadotropic hypogonadism, hypogonadotropic hypogonadism, hirsutism, virilization and luteal phase deficiency. TSH is used to exclude hypothyroidism.

Patient Preparation:
Specimen collection after fluorescein dye angiography should be
delayed for at least 3 days. For patients on hemodialysis, specimen
collection should be delayed for 2 weeks. According to the assay
manufacturer Siemens: "Samples containing fluorescein can produce
falsely depressed values when tested with ADVIA Centaur TSH3 Ultra
assay."

Collect 3-4 hours after patient has awakened.
Overnight fasting is preferred.

Note: Do not order progesterone testing on a barrier gel
specimen older than 72 hours.

Prolonged contact of the serum with the barrier gel can
cause decreased progesterone results.

DHEA supplementation causes a false elevation of values in this
standard immunoassay method for progesterone. Physicians using
progesterone levels as a criterion for triggering ovulation in an
IVF cycle in patients supplemented with DHEA should assess
progesterone levels using a LC/MSMS Progesterone assay, such as
test code 17183 performed at Quest Diagnostics San Juan Capistrano.

Rejection Criteria: Progesterone Serum stored on barrier gel for more than 72 hours; plasma Transport Temperature: Room temperature According to the assay manufacturer Siemens: "In general, the fluorescein clearance (requires) about 48 to 72 hours." For patients in "end stage renal disease on hemodialysis, the elimination will be delayed likely by several cycles of hemodialysis. Assuming the dialysis cycle serves as one half-life (and) using the rule of thumb that 5 half-lives are required to completely eliminate a compound, two weeks is conservative (if) dialysis is performed 3 times per week."
