Specify antimicrobial(s) to be tested and those used to treat
patient.
To prepare isolate:
1. Culture on a blood agar slant or appropriate bacteriological
media. Wrap Parafilm around the screw cap or plate and wrap tube
in paper towel to protect the glass tube from breaking inside
the outer container or wrap plate and transport in cardboard box.
2. Place the properly labeled tube or plate into appropriate
sized container. On the outer container affix a label
"Etiologic Agent/Biomedical Material."
3. Ship specimen by overnight courier (following
courier regulations) at ambient temperature.

If sending isolate, specify ID of organism and source of the specimen. SEND PURE CULTURE ONLY. If the isolate submitted is mixed, there will be no workup done pending clarification with the client. There will be separate charges for each organism that requires bacterial identification and sensitivities. If the isolate submitted needs identification, sensitivities will be delayed until the identification is complete.
